Leading NFPs collaborate to create Dementia and Stroke or Coronary Heart Disease Toolkit

More than 332,000 Australians live with dementia and many of these individuals also live with stroke or coronary heart disease. The collaborative toolkit is designed to support better health outcomes and improve support for these people.

“A person with dementia may have trouble telling you about the pain associated with a separate condition. They may have issues around remembering to take their medication, issues with physical activity and are more likely to develop depression or anxiety,” said Dr David Sykes, General Manager Learning and Development at Alzheimer’s Australia Victoria.

The toolkit includes information on the causes, symptoms and strategies for managing the conditions, and is accompanied by several case studies and three educational videos and is suitable for the person with the diagnosis, their carers and community care workers.
